Frequently Asked Questions
Am I eligible?
The Elevate program is only available to those with Medicare health insurance coverage at this time. Please inquire as not all Medicare plans are eligible.
Where are your offices located?
Our Fountain Valley office is conveniently located on the campus of MemorialCare Orange Coast Medical Center (9900 Talbert Ave in Suites 301 and 302). Our Huntington Beach office recently relocated to a newly renovated, standalone facility at 20981 Magnolia Street (on the corner of Magnolia St. and Atlanta in the Landmark Plaza shopping center).
Do all doctors practice at both locations?
No. There are a select few doctors who do not travel to both offices. Please feel free to contact us if you have a physician and location preference.
Can I easily change my doctor?
You may select and change your doctor based on availability. If your desired doctor is not accepting new Elevate members, you may join their waitlist and see another doctor in the meantime.
Can I visit any hospital or specialist?
The answer to this question is based on your health plan type. HMO patients require a referral from the designated primary care doctor, and they must visit an in-network specialist or hospital. Patients with a PPO plan may visit any hospital or specialist; however, these patients will not be cared for by an Edinger Medical Group doctor at MemorialCare Orange Coast Medical Center in the event they are hospitalized. Hospitalization is only available to HMO patients.
