A Message from Dr. Fogarty:
Dear Patient,
It’s a pleasure to be chosen as your Primary Care Provider and I look forward to caring for you for many years to come. In the event that I am unavailable, I have the utmost confidence that my Nurse Practitioner partners, Rebecca Lee, FNP and Lana Nguyen, FNP, will be able to assist you should you have any urgent requests, questions or health concerns. I hope that you will consider them an integral part of my treatment team. It is an honor to be part of such a reliable group of physicians and nurse practitioners who are willing to extend their care and attention to my patients when needed.
