A Message from Dr. Middlebrooks:
Dear Patient,
It’s a pleasure to be chosen as your Primary Care Provider and I look forward to caring for you for many years to come. In the event that I am unavailable, I have the utmost confidence that my Nurse Practitioner partner, Victor Vo, AGNP, will be able to assist you should you have any urgent requests, questions or health concerns. I hope that you will consider him an integral part of my treatment team. It is an honor to be part of such a reliable group of physicians and nurse practitioners who are willing to extend their care and attention to my patients when needed.
