Steps to Enroll as a New Patient
Step 1: Determine if We Are the Right Fit
Before enrolling, we encourage you to learn more about our provider team and services. Visit our About Us page to explore our approach to patient care.
Step 2: Verify Your Insurance Coverage
We accept a variety of health plans. To confirm if your insurance is accepted:
- View our list of accepted health plans here.
- Contact your health plan provider to verify your specific coverage.
- Call our billing department at 714.965.2500 (available Monday – Friday, 8:30 a.m. to 5:00 p.m.) for assistance.
Step 3: Schedule Your First Appointment
To book your first in-office visit:
- Call our scheduling department at 714.965.2500 (press option 1).
- Text our main office line to communicate directly with an EMG team member.
Step 4: Complete Your Patient Registration
Before your first visit, complete our electronic patient registration forms online. Click here to access the forms.
Be sure to provide a valid email address, as this will be used to access your MyEMGChart Patient Portal, where you’ll receive important updates about your health and our practice. Learn more about the Patient Portal.
What to Bring to Your First Appointment
To ensure a smooth visit, please bring the following:
- A valid photo ID.
- Your insurance card.
- Your prescription card (if applicable).
Plan to arrive 15 minutes early for parking and check-in. If you anticipate being more than 15 minutes late, please call or text us at 714.965.2500 to reschedule if necessary.
