What to bring to your first appointment

We kindly ask that you bring your ID and insurance card to your first office visit.

If you received the COVID-19 vaccine, we encourage you to also bring a copy (front and back) of your vaccination card for our records.

According to the CDC, masks are still required to be worn by all patients in healthcare facilities – fully vaccinated or not. You must wear a mask to your appointment and keep it on for the duration of the visit until further notice. The EMG Team thanks you in advance for your cooperation!
